Providing care to a loved one is a full time job. At St. Luke, we know the demands of providing care, we are experienced professionals who do so on a daily basis. Often, caregivers in our community who are responsible for meeting the needs of a family member or friend need respite or someone else to provide care from time to time, but are unsure of where to turn. Adult Day Health Care is a "medical model" adult day care program. Registrants receive daily health monitoring and activities program designed to promote independence and wellbeing for adults.

The program serves individuals age 18 years and older who require on-going rehabilitation and medical services in an atmosphere that is supportive and friendly.

Open Monday through Friday, 10:00am - 4:00pm.

How to Access St. Luke Adult Day Health Care

Adult Day Health Care at St. Luke serves all of Oswego County. Because we are centrally located in the city of Oswego and easily accessible, local medical transport agencies, family or friends can bring registrants to us or we can arrange to have your loved one picked up and returned home. Adult Day Health Care is a Medicaid eligible program.
