Providing care to a loved one is a full time
job. At St. Luke, we know the demands of providing care, we
are experienced professionals who do so on a daily basis.
Often, caregivers in our community who are responsible for
meeting the needs of a family member or friend need respite
or someone else to provide care from time to time, but are
unsure of where to turn. If you want to ensure that your loved
one recieves the care and more, turn to St. Luke.
Adult Day Health Care is a "medical model" adult
day care program. Registrants receive daily health monitoring
and activities program designed to promote independence and
wellbeing for adults.
The program serves individuals age 18 years
and older who require on-going rehabilitation and medical
services in an atmosphere that is supportive and friendly.
Open Monday through Friday, 10:00am - 4:00pm.

Registrants in our Adult Day Health Care Program
are offered a variety of services that are individualized
to meet specific needs. Personal care services range from:
- health and medical care, including case management
- health assessment
- physical rehabilitation therapy, occupational therapy,
speech therapy
- dental services
- therapeutic diets, nutritional education and more.
In addition to the full array of medical services
available, registrants can also involve themselves in the
many varied activity programs that take place at St. Luke
throughout the day. These activities are designed to keep
registrants engaged and involved while they receive the finest
health care in our area.
How to Access St. Luke
Adult Day Health Care
Adult Day Health Care at St. Luke serves all
of Oswego County. Because we are centrally located in the
city of Oswego and easily accessible, local medical transport
agencies, family or friends can bring registrants to us or
we can arrange to have your loved one picked up and returned
home. Adult Day Health Care is a Medicaid eligible program.
